Air Force: Pilot Error Caused Helicopter Crash

The Air Force said today that pilot error caused the crash of an HH-60G rescue helicopter last October near Patrick Air Force Base, FL. The HH-60, assigned to the 41st Rescue Squadron at Moody Air Force Base, GA, was the lead aircraft of a two-ship formation during a combat search and rescue exercise over the Kissimmee River on Oct. 14, 2001 when the helicopter crashed, the Air Combat Command Accident Investigation Board report reads.
